History Consultation

As a new client your first visit will entail a History Consultation and Examination. You will receive a personal history document to fill in and then Dr Sandy (Chiropractor) will ask you some questions to help get a complete picture of your health. It may be helpful to bring a list of your past treatments, if any and previous X-rays to this first consultation. This will allow us to determine your individual concerns and needs and to assess any past injuries and treatments that may be relevant to your current condition.
